Medications

Clinician-guided medication options for weight management. Vanguard Performance LLC operates the brand and merchant service; Architek Health provides branded telehealth technology, independent licensed clinicians make medical decisions, and licensed pharmacies dispense prescribed medicines.

Weight management

Tirzepatide

Compounded medication, when prescribed, is not FDA approved. Tirzepatide is a prescription medicine that acts on GIP and GLP-1 receptors involved in appetite and food intake. Our weight-management pathway starts with a clinician’s review of your goals, medical history, medicines and individual risks. The clinician decides whether tirzepatide is appropriate and, if prescribing, selects the preparation and treatment plan. Ongoing use and changes require clinical direction; choosing this pathway does not buy a prescription or a promised amount of weight loss.

Tirzepatide can cause gastrointestinal effects, including nausea, vomiting, diarrhea and constipation, and has important warnings and contraindications. Tell the clinician about pregnancy or pregnancy plans, other medicines, and any personal or family history of medullary thyroid carcinoma or MEN 2. Read the full information for the medicine actually prescribed. Compounded medicines are not FDA approved. FDA does not assess their safety, effectiveness or quality before marketing. They are not FDA-approved generics or interchangeable substitutes for approved medicines. A clinician must determine whether a compounded preparation is appropriate; the pharmacy is responsible for its preparation and labeling.

Eligibility requires a licensed clinician’s evaluation. A prescription and results are not guaranteed.

Semaglutide

Compounded medication, when prescribed, is not FDA approved. Semaglutide is a prescription medicine that acts on GLP-1 receptors involved in appetite and food intake. A licensed clinician reviews whether it fits your health history, current medicines and weight-management goals, and determines any preparation, prescription and follow-up plan. Medical review can also result in a different recommendation, further evaluation or no prescription. Do not start, combine or change medication on the basis of a product page.

Microdose tirzepatide

Architek’s published microdosing model uses compounded medicines and clinician-controlled low doses. This tirzepatide evaluation pathway requires a clinician to assess whether treatment is appropriate and select any patient-specific preparation and dose. Tirzepatide acts on GIP and GLP-1 receptors involved in appetite and food intake. “Microdose” is not a standardized FDA-approved regimen or a promise of fewer side effects. Do not infer a dose or schedule from this name or change treatment without clinical direction.

The risks and contraindications of tirzepatide still matter when a smaller dose is considered; a smaller dose is not a safety guarantee. Discuss pregnancy, other medicines, and personal or family history of medullary thyroid carcinoma or MEN 2 with the clinician. Compounded medicines are not FDA approved. FDA does not assess their safety, effectiveness or quality before marketing. They are not FDA-approved generics or interchangeable substitutes for approved medicines. Weight management

Metformin

Metformin affects glucose production by the liver and the body’s response to insulin. Weight-management use is off-label: metformin is not FDA approved specifically for weight loss. This pathway provides clinical assessment of whether that use is appropriate for your circumstances, rather than automatic approval based on a purchase. The clinician reviews your medical history, kidney function, other medicines and relevant risks, and decides whether treatment and monitoring are appropriate.

Metformin commonly causes gastrointestinal effects and carries a serious warning for lactic acidosis. Kidney problems and certain other conditions or medicines can increase risk; tell the clinician about kidney or liver disease, alcohol use, planned procedures and every medicine you take. Follow the prescribed product’s instructions. Off-label use does not establish that a particular weight-loss result will occur.

Eligibility requires a licensed clinician’s evaluation. A prescription and results are not guaranteed.

Care begins with a clinician

The Architek pathway is for adults 18 and older. Architek reports a licensed clinical network covering all 50 U.S. states and confirms GLP-1 prescribing coverage subject to applicable law. Your actual location, the treating clinician’s authorization, clinical suitability and the medicine/preparation and pharmacy’s service limits must be checked before any charge. This is self-pay care; the clinical service does not bill insurance. Under Architek Health Provider Network PC’s published eligibility policy, patients covered by Medicare, Medicaid or TRICARE are ineligible for this clinical pathway. Private insurance does not make this an insurance-billed service. Evaluation may be asynchronous or require a live visit; no prescription or result is guaranteed.

Not for emergencies. Call 911 or seek emergency care for a medical emergency.

How the program works

  1. 1. Choose a clinical evaluation

Select the medication pathway you want to discuss. The corresponding Architek survey takes you to the brand’s clinical intake, where you provide your own identity, location and relevant health history and review the applicable patient notices and consents. Product choice is a request for assessment, not a treatment decision.

  2. 2. An independent clinician reviews

Your questionnaire and messages are reviewed when a clinician is next available. Where permitted, review is asynchronous rather than an immediate live consultation. Timing depends on complete information, follow-up questions and whether state rules or clinical circumstances require a phone/video visit or an in-person assessment. Include medicines, allergies and relevant conditions and answer follow-up requests. The clinician may recommend another assessment or decide that remote care or medication is not appropriate. Routine laboratory testing is not automatically required for every GLP-1 review; the clinician can request it when appropriate.

  3. 3. Review the decision and any prescription

The clinician determines whether to prescribe and explains the treatment plan, material risks and follow-up. A licensed pharmacy handles dispensing under the prescription and supplies the medicine’s directions. Vanguard does not prescribe, compound or dispense. Clinical approval and pharmacy fulfillment are separate steps; a completed questionnaire is not a prescription or shipment confirmation. Architek describes routing prescriptions within its pharmacy network according to location, pharmacy capacity and preparation availability. The particular dispenser is identified with the prescription and fulfillment information.

  4. 4. Continue care through the clinical service

Use the clinical service’s contact instructions for treatment questions, side effects, dose changes and refill requests. Renewals and changes depend on clinical review, not on continued payment alone. Architek delivers branded clinical service communications. For merchant billing or website support, use our Contact page. For an emergency, call 911; a routine survey or support email is not monitored as emergency care.

Questions about care and the service

Does payment guarantee treatment?

No. A medical evaluation can result in no prescription, a request for further assessment or a different recommendation. The consultation is distinct from medication dispensing. Review the stated fees, inclusions and cancellation/refund conditions before authorizing a charge; clinical eligibility and billing are separate decisions.

Who is responsible for my care?

An independent licensed clinician is responsible for medical evaluation and prescribing, and a licensed pharmacy is responsible for dispensing. Architek provides the branded telehealth technology and clinical communications. Vanguard Performance LLC operates Vanguard and Filia and acts as merchant. Our support team cannot change a prescription or replace clinical advice.

How do I prepare for review?

Provide accurate identity and current physical location, health history, allergies and a complete medicine list, including other weight-management medicines. Tell the clinician about pregnancy, pregnancy plans or breastfeeding and any relevant serious condition. Do not stop prescribed treatment to use this service without speaking with your treating clinician.

How do I raise a billing, privacy or care concern?

For a charge, cancellation request, website issue or privacy request concerning Vanguard, use the business email on the Contact page and provide only the minimum information needed. Use the treating practice or dispensing pharmacy’s secure contact route for clinical questions or medical records. We can help route a concern, but ordinary email is not a secure clinical portal.

Where is care available?

This evaluation pathway uses Architek’s licensed clinical network, which Architek reports covers all 50 U.S. states. Architek has confirmed that its provider group can prescribe GLP-1 medicines to patients in all states, subject to applicable law, and that pharmacy partners serve California. The actual prescriber must be authorized for your current physical location. Availability of a particular medicine, preparation and dispensing pharmacy depends on state requirements and pharmacy routing and must be checked before any charge. Nationwide network coverage does not promise every medicine or formulation can be prescribed or shipped everywhere.

When will my evaluation be reviewed?

A clinician reviews your questionnaire and messages when they are next available; asynchronous review is not an immediate appointment or emergency service. Timing varies with how complete the information is, follow-up questions and whether a live phone/video visit or another assessment is needed. A decision may therefore require more than the initial questionnaire. Pharmacy preparation and shipping occur only after a prescription and are separate from clinical review. Use the clinical service’s contact instructions for a pending evaluation; do not wait for routine review in an emergency.
